New Passport
The perfected application form number 'K' which can be obtained from the Consulate free of charge or can be downloaded from the website Forms section.
The application for new passport should accompany the following documents and the applicant must come in person to submit it.

Three passport size color photographs.

Original existing passport with a Photocopy.

Application for all country passport should be submitted with a copy of the birth certificate or copy of National Identity card.
Lost Passport
In case of lost Passport, a Police report should be submitted along with Legal English translation and attested by the Ministry of Foreign Affairs, U.A.E.
In case of damaged passport, the damaged passport should be submitted along with the application with the Sri Lanka Identity card if available or Citizenship certificate if the applicant is a citizen by registration.
Separate Passport for Children
In case of applying for a separate passport for children under the age of 16 years, the following original documents should be submitted along with the application.

Written consent or no objection letter from the parents.

Marriage certificate of parents.

If the child is born abroad section 5(2) certificate of Citizenship.

Children should be present at the Consulate while handing over their application.
Processing of a new passport will take between 30 to 45 days.
Renewal and Amendments of Passport

The perfected & signed application for renewal and amendments will be accepted from 08.30hrs to 13.00hrs.

The application form can be obtained from the Consulate free of charge or downloaded from Forms section .The form number is 'O'.

The application for renewal should be submitted along with the existing orignal passport.

The application for amendments in the passports require following documents (as per type of amendment) with the application.

To change/add the name of a lady in the passport after marriage or divorce the original marriage or divorce certificate is required.

To change/add the occupation or job description in the passport a letter of appointment is requied or a Professional/Academic certificate (attested by the respective authorities) should be submitted.

Application for inclusion of names of minor children**
**For the inclusion of names of minor child or children under 16 years of age in parent’s passport the perfected form number 'C' should be accompanied with the following documents. The application form can be obtained from the Consulate free of charge or downloaded from Forms section .

Three passport size color photographs. Original Birth certificate of the child.

If the child born abroad section 5(2) certificate of citizenship.

Original Marriage certificate of the parents.
-
Attested relevant pages of the spouse’s passport or copies with original passport.
-
Consent letter from the spouse.
-
Court order if the spouse is a divorcee.
-
Death certificate of the spouse in case of widow or widower.
It is necessary to bring the children when handing over the application.
